HEALTH PLAN

Medical Coverage
Tevis offers two options of coverage to meet the varying needs of our eligible employees and their families:
• Point of Service (POS) plan
• Health Maintenance Organization (HMO) plan
• Both plans offer a Prescription Plan and have a Wellness and Prevention Program.

Dental Coverage
Tevis also offers a choice of coverage for our eligible employees and their families:
• Indemnity (Freedom Advance PPO) plan
• Dental Health Maintenance Organization (DHMO) plan

Vision Coverage
Vision benefits include annual eye exams with discounts on eyewear and Lasik surgery.

Life Insurance
We provide a basic life insurance benefit of $20,000.00 for each eligible employee, fully paid by the company.

Accidental Death & Dismemberment (AD&D) Insurance
A rider is included with our life insurance plan. This feature is also fully paid by the company.

SAVINGS AND RETIREMENT PLAN

Profit Sharing
Employees have a real impact on the profitability of the company. To recognize and reward those efforts, we offer a Profit Sharing Plan to eligible employees. The amount of the company’s contribution to the plan is based each year on the company’s profits.

401(K) Qualified Retirement Plan
We provide eligible employees with a 401(K) plan. The company’s contribution is a 25% match of the employee’s salary deferral, up to 10%. Participants may elect a pre-tax deduction up to the IRS maximum each year.
